S:US is seeking a Senior Case Manager for their homeless prevention program-Home Base.
This position is located at W. 181 Street New York, NY 10035
SCOPE OF ROLE:
Under the supervision of the Program Supervisor, the Senior Case Manager ensures quality service
delivery to individuals and families in need of homeless prevention services.
They will provide supervision to case managers, oversee intensive case management for program participants and their families which include arranging, coordinating, monitoring and delivering of services related to meeting their housing needs. Assure timely intakes, assessments, service plans and referrals.
Participate in development of After-Care and on-going housing assistance service planning processes. Coordinate supportive services and referrals based on specific needs of program participants. Monitor case progress and when necessary enter information into the CARES database.
REQUIRED EDUCATION AND EXPERIENCE
• Bachelor’s Degree is Human Services field required.
• 3+ years’ experience with homeless population.
• Ability to work with a culturally diverse population.
PREFERRED QUALIFICATIONS & SKILLS
• Master’s degree in human services or related field.
• Familiarity of NYC public school system preferred.
• Experience in residential setting, group work, tenant organization, case management, referral, and/or community outreach strongly preferred.
