Position Overview

The Residential Habilitation Specialist ensures that necessary residential habilitation is provided in an effort to assist in maximizing independence and integration in the community for individuals living within an Individualized Residential Alternative.

  • Develop Residential Habilitation Plans in accordance with services outlined in each person’s Individualized Service Plan
  • Develop formal goals as needed to assist individuals in acquiring skills necessary to meet their valued outcomes
  • Provide in-service training to staff on how to implement plans, collect necessary data, and turn over data sheets on a monthly basis
  • Write a monthly Residential Habilitation note for each individual
  • Attend Semi-Annual and Annual ISP reviews and provide a written report for Semi-Annual and Annual ISP reviews regarding the Residential Habilitation being provided to each individual and the progress being made 

Qualifications

REQUIRED EDUCATION AND EXPERIENCE

  • At least a Bachelor’s Degree in a human services field
  • At least 2-years experience collaborating with people with intellectual and developmental disabilities

 

PREFERRED QUALIFICATIONS & SKILLS

  • Able to use Microsoft Word; Microsoft Excel; PowerPoint with proficiency
  • Proficient writing and organizational skills
  • Excellent time management to complete multiple tasks simultaneously
  • Experience writing formal valued outcomes/goals and/or Staff Action Plans preferred
  • SCIP-R trained
