Position Overview

SCOPE OF ROLE:


The Developmental Disabilities Division believes that our role in supporting people with Intellectual and
Developmental Disabilities is to live meaningful lives as determined by the person. All persons shall live a

and a clean and safe home.


Our Residential Habilitation Specialist is responsible for ensuring that the necessary residential
habilitation is provided to the persons supported using a person-centered approach. In this role, the
specialist will make every effort to assist the persons supported, living within an Individualized Residential
Alternative (IRA) setting, to maximize independence and integration in the community. The selected
candidate will lead teams in creating meaningful activities and valued outcomes for the persons served in
the areas of - community integration; socialization; recreation and mindful motivation. They will
promote, research and provide activities that further enrich people’s lives, while providing options in both
residential and community environments. The Residential Habilitation Specialist, in times of life event
changes and circumstances, will display a willingness to trouble-shoot and support teams to develop
creative ways for continued compliance. The candidate will be a true champion of person-centered
person served.


II. ESSENTIAL DUTIES & RESPONSIBILITIES:
Life Plans
• Develop tools and activities for engagement as it pertains to person-centered approaches and quality of
life for persons served.
• Develop Staff Action Plans (SAP) in accordance with services outlined in each person’s Life Plan by
developing valued outcomes that are person-centered, meaningful and meet the wants and needs of the
person served.
• Create activity schedules and Interim Day Habilitation plans for persons served who do not attend a Day
Habilitation Program.
• Utilize feedback and input from the person served and their Circle of Support when creating activity
schedules.
• Ensure Life Plan dates and content are accurate and finalized following the Life Plan review procedure.
• Upload finalized Life Plans and corresponding documents to the Precision Care system and distribute Staff
Action Plans to all parties within a timely manner.

• Check the Precision Care calendar on a daily basis—to ensure timeliness of certification of Staff Action
Plans in order for Residential Habilitation data collection to be captured accordingly.
• Develop Residential Habilitation/Community Habilitation Staff Action Plans as it pertains to emergency,
unforeseen circumstances, which includes, but is not limited to the training of staff and management;
preparing consents and addendums in accordance to revised regulations with appropriate valued
outcomes to correlate with the situation (i.e.: COVID-19 Group Community Habilitation-R).
Collaboration
• Work closely with clinical and management staff regarding the development and implementation of
person-centered valued outcomes and goals.
• Work closely with QA department on issues that can potentially affect billing compliance standards are
identified.
• Build relationships with Care Coordination Organizations (CCO) in order to ensure billing compliance
adherence within S:US.
• Participate in learning collaboratives and trainings to become team leaders; project leaders, person centered planners and facilitators for persons served as it relates to self- direction and person-centered
supports and practices.
• Develop working relationships with OPWDD and local DDRO’s as it pertains to person-centered practices
and self-direction
• Develop working relationships with coordinators at Meaningful NY as it pertains to Self-Direction
• Develop working relationships with Care Coordination Organizations
Training & Networking
• Provide ongoing training and support to new/veteran staff and management in regards to compliance and
person-centered practices through observations; role modeling and problem-solving.
• Provide ongoing training to staff and management on plans as well as when billing issues are identified.
• Attend internal and external trainings and implement tools and materials within the division in the areas
of person-centered approaches and self-direction.
Reporting
• Write monthly progress notes for each person served, detailing the progress of each valued outcome,
making suggestions and recommendations to the person and their Circle of Support as deemed
appropriate.
• Review community inclusion and daily DSP notes to include pertinent information in monthly progress
notes.
• Review residential habilitation data collection and interpretation of data on a weekly basis, ensuring all
appropriate documents are readily available in order to ensure billing standards and compliance.
• Attend all Life Plan meetings and advocate for the person’s wants and offer support to the person so their
voice is heard.
• Share valuable information based on observations, conversations and review of data collection to report
on progress.
• Provide a written report regarding the Residential Habilitation being provided to each person and the
progress being made.
• Follow-up with Care Coordination Organizations within 2 weeks of Life Plan meetings, requesting plans as
well coordinate Life Plan meeting dates.

 


• Participate in Clinical/Management team meetings where valued outcome progress is reviewed and
discussed.
• Collaborate closely with the clinical team—review CSET assessments and determine if valued outcomes
are necessary in various areas of human development
Additional responsibilities as deemed necessary and/or appropriate by the Director of Individualized
Supports.

• Self-Motivated: Ability to demonstrate initiative and endeavor to maintain a stable level of
productivity

Qualifications

• At least a Bachelor’s Degree in a human services field
• At least 2-years experience working with people with intellectual and developmental disabilities


SPECIAL SKILLS
• Able to use Microsoft Word; Microsoft Excel; PowerPoint with proficiency
• Good writing and organizational skills
• Excellent time management in order to complete multiple tasks simultaneously
• Experience writing formal valued outcomes/goals and/or Staff Action Plans preferred
• SCIP-R trained
