Position Overview

Services for the UnderServed (S:US) isseeking a Senior Diversion Specialist for their homeless prevention program.

 

This position is located at 400-430 East 30 Street New York, NY 10035.

 

SCOPE OF ROLE:

Coordinate intake and referral process for adult diversions at DHS identified shelters.

 

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Assess shelter applicants directly referred from single and adult family intake centers and serve as the first point of contact.
  • Promote linkage development and monitor the effectiveness of linkages with other housing service providers.
  • Provide service description and overview to prepare participants for other housing options other than shelter.
  •  Review documentation of diversion and non-diversion clients before submission to the supervisor.
  •  Act as the liaison between the shelter Director and Diversion Supervisor.
  • Maintain regular contact and complete requested tasks from the Diversion Program Supervisor, Homebase Program Supervisor, Homebase Assistant Program. Director, and Homebase Program Director.
  •  Enter and maintain referral and intake information in a timely manner.
  • Explore alternatives to shelter (reuniting with family/friends, return to previous living situation, refer to relevant treatment facilities, etc.)
  •  Develop individualized housing plan (also referred to as an Initial Service Plan).
  •  Conduct home visits as needed.
  •  Provide general case management including coordination and delivery of services relating to housing stability.
  •   Entitlement referral and advocacy.
  •  Meet with participants as needed for follow-up and recommendations.
  •   Meet daily targeted quota for program.
  •   Assist participants in identifying, locating, obtaining, and retaining suitable housing.
  •   Participate in meetings to promote the program.
  •  Cultivate, develop and maintain productive relationships with referral agencies that are related to retaining housing and service providers.
  •  Actively participate in internal and external training programs and committees.
  •  Ensure program participants’ rights are protected.
  • The essential functions of the job include but are not limited to the job description.  Tasks may be modified, expanded and or assigned over time.

Qualifications

  • Bachelor’s Degree in Human Service or related field or,
    High School Diploma or GED with a minimum of 2 years applicable experience

 

 

PREFERRED QUALIFICATIONS & SKILLS

  • The candidate should be able to read documents for analytical purposes such as computer information, software, computer graphic design programs, etc
  • Limited applications of manual dexterity and hand-eye coordination
