Position Overview

S:US is seeking a Sr. Case Manager for their mens shelter in Brooklyn.

This position is located at 1000 Blake Avenue Brooklyn, NY 11208.

 

The Senior Case Manager will oversee the Case Managers and Housing Specialist.  He/she will provide supervisory oversight and guidance to Case Manaagers  in the development of initial service plans and service plan reviews, referring clients for in-house and community-based medical, mental health, and supportive social services, and referring clients for in-house and community-based medical, mental health, and supportive social services.  Will provide oversight and training to Case Managers in assisting clients with the financial entitlement and medical benefit application process.  The Senior Case Maanger will provide supervisory oversight and training to Case Managers and Housing Specialists in group and individual counseling, workshops, and recreational activities.  Will schedule and assign groups and activities to Case Managers as needed, and run groups as necessary. He/she will Manage program census and census data: Manage the intake process; assign intakes to Case Managers. Perform full intake and assessment with new clients within 24 hours of admission as needed.

Qualifications

  • Bachelor degree in a Human Service field required. Master’s Degree preferred.
  • A minimum of two years’ experience working with chronically homeless individuals with co-occurring disorders.
  • Ability to work independently and as part of a team, to assist a population that is diverse in ethnicities, backgrounds, ages, and sexual preferences. Bilingual preferred. CPR and First Aid certification preferred.
