S:US is seeking a licensed Senior Social Worker for our Supportive Crisis Stabilization Center at 2862 Fulton Street in Brooklyn.

 

Available schedules: 10am-6pm, 12am-8am, 8am-4pm, 4pm-12am

Two evening shifts per week and availability to work on weekends

Educational Requirements: LCSW, LMHC  (Required)

 

Essential Job Functions:

  • Provides clinical treatment in accordance with scope of practice and regulations governing 14 NYCRR (Part 600 regulations and other applicable laws and regulations).
  • Actively monitors the integrity of consumer data; responsible for the reliability and accuracy of clinical data housed by SUS' Electronic Health Record (EHR) and Practice Management software; documents all services in accordance with SUS policies, NYSOMH regulations and all relevant federal laws; maintains the strict confidentiality of consumers currently engaged in or formerly a recipient of treatment services.
  • In coordination with relevant members of the SUS management team, conducts limited outreach and marketing to potential referral sources internal and external to SUS; active liaison with community service providers, hospitals, and other prospective referral sources is may be required.
  • Supervise Social work and peer staff
  • Participates in client(s) discharge planning, collateral contact and safety planning.
  • Establish connections with MCO’s regarding notification sheets, follow ups, clinicals and discharged clinicals for Crisis stay
  • Attends staff meetings, in-serving training, case conferences, and weekly clinical meetings.
  • Provide individual and group counseling (Mental Health & Substance Use D/O) as needed
  • Provide Crisis intervention and clinical guidance as appropriate
  • Provide weekly and monthly reports
  • Maintain program data to ensure program compliance
  • Make community referral recommendations as appropriate to (LOC) level of care and community resources
  • Provide strength based, Person centered, trauma-informed, Social determinants of health assessments of children’s biological, educational, psychological and social needs.
  • Provides emergency and relief coverage as necessary.
  • On -Call 24 hrs a day / 7 days a week 

 

Qualifications

Job Requirements:

  • Master degree
  • Educational Requirements: LCSW, LMHC  (Required) 
  • Minimal 5 years ’supervisory experience
  • Minimum 5 years clinical experience.
  • Ability to work evenings and/ or weekends
  • Knowledge of substance use and mental illness as it relates to working with families, Youth/children and adults.
  • Effective interpersonal and written communication skills and ability to work as a member of an interdisciplinary team; able to relate with diverse staff; customer service focus in interactions with diverse client population.
  • Ability to document and review professional clinical notes within appropriate timelines and input in EHR system.
  • Experience working with people diagnosed with mental illness and people struggling with substance use and Co-Occurring disorder.
  • Experience working within a respite/residential or crisis 24 hr program
  • Effective written and oral communication skills.
  • Computer literacy skills (Excel, Teams, Windows, Outlook)
  • Experience working with children/youth and /or Families

 

PREFERRED QUALIFICATIONS & SKILLS

  • Bilingual English/Spanish speaking preferred
  • Possession of Supervision in Field Instruction (SIFI) (Must secure within 1 yr of employment)
