Position Overview

REPORTS: OnTrack Team Leader 

 

The salary min is $65,000 and the max $67,600.

This position is located in Brooklyn, New York. 

 

REQUIREMENTS: 

  • LMSW, LCSW, LMHC, LCAT required.
  • Experience with outreach to underserved populations and direct service to adolescents and young adults with serious mental illness.
  • High degree of computer literacy
  • Experience in an Article 31 Mental Health Clinic preferred
  • Familiarity with recovery-oriented, evidence-based clinical practice preferred
  • Experience with dually diagnosed individuals preferred
  • Demonstrated experience with diverse populations preferred
  • Bi-lingual Spanish preferred
  • Experience in community-based mental health services

          

JOB FUNCTIONS:        

  • The OnTrackNY Program is a Coordinated Specialty Care (CSC) treatment program at serving adolescents and young adults who are within two years of experiencing psychosis. Our mission is to keep young people on track with their goals for school, work and personal relationships. Equity, inclusion, rapid access and self-determination are at the core of everything we do. OnTrackNY team staff receive expert training and technical assistance in the CSC model and protected time for professional development through the statewide OnTrackNY Initiative at the Center for Practice Innovations at Columbia Psychiatry and New York State Research Foundation for Mental Hygiene.
  • The Primary Clinician/Outreach and Recruitment Coordinator will work as a part of a multidisciplinary team to provide coordinated services, through recovery-oriented and evidence-based interventions, to strengthen autonomy, self-efficacy, resiliency, and overall functioning in early stages of illness.
  • Provides clinical treatment in accordance with scope of practice and regulations governing Article 31 outpatient mental health clinics (Part 599 regulations and other applicable laws and regulations). They will maintain consumer information in the electronic health record and coordinate services with other providers.
  • Services are offered at the clinic site, via telehealth, or off-site in the community, as determined by participant and family needs.
  • Ongoing education and training is provided to all team members pertaining to first episode psychosis (FEP) and clinical models and interventions to best support individuals with recovery.

 

RESPONSIBILITIES:

  • Provide full range of therapeutic interventions including needs assessments, safety planning, psychoeducation, resource provision, Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and family therapy.
  • Use Shared Decision Making framework to support participants with ongoing treatment needs.
  • Provide psychoeducation pertaining to first episode psychosis (FEP) and mental health recovery consistent with the goals of OnTrack.
  • Provide services in the community (off-site) as needed based on participant and family preferences.
  • Enrich family and participant with resources to support stabilization through discharge.
  • Collaborate with OnTrackNY team members to set new community-based goals focused on reducing inequities and barriers to accessing specialty care.
  • Facilitate referral processes including monitoring referrals and conducting eligibility screenings.
  • Establish collaborative relationships with community partners who might refer or provide ongoing support to young people and families.
  • Serving as primary contact person for referral sources and maintaining on-going communication with referral sources
  • Referring participants ineligible for services to other appropriate community providers
  • Establishing and maintaining database of referral sources with up-to-date contact information.
  • Rotate on-call responsibilities with other team clinicians.
  • Identifying and creating new resources to support diverse young people and families seeking or receiving services at OnTrackNY
  • Ensuring a welcoming and efficient process for young people and families seeking care in OnTrackNY
  • Spreading awareness about OnTrackNY services through community education and outreach activities that emphasize OnTrackNY’s uniquely flexible and youth-friendly approach
  • Attending weekly team meetings
  • Participating in ongoing training and supervision
  • Serve as the Primary Clinician for a subset of program participants. In this role as Primary Clinician, the Outreach and Recruitment Specialist will: 
  1. Serve as the point person for the participant and their family;
  2. Provide services in the community (off-site) as needed based on participant and family preferences.
  3. Conduct needs assessments;
  4. Engage in safety planning and wellness management planning with participants;
  5. Connect participants and families with the services and supports they need;
  6. Provide behavioral interventions and take an especially active role in the family support domain.
  7. Conduct clinical assessment to determine the participant’s level of social functioning and substance use via a shared decision making (SDM) framework
  8. Provide Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and Psycho-education.
  9. Provide psychoeducation regarding first episode psychosis (FEP) and the goals of the OnTrackNY program;
  10. Provide supportive therapy;
  11. Develop and modify treatment using a shared decision-making process with clients
  12. Work with participants and family for transition from the team to other services and supports, and ensure stable links at discharge.
  • Actively monitors the integrity of consumer data; responsible for the reliability and accuracy of clinical data housed by SUS' Electronic Health Record (EHR) and Practice Management software; documents all services in accordance with SUS policies, NYSOMH regulations and all relevant federal laws; maintains the strict confidentiality of consumers currently engaged in or formerly a recipient of treatment services.
  • Utilizes best and evidence-based approaches to treatment consistent with S:US' organizational culture; health/mental health integration, rehabilitation and recovery, and wellness approaches figure prominently.
