Position Overview

The Case Manager provides and coordinates family services. 

 

  1. ESSENTIAL DUTIES & RESPONSIBILITIES:
  • Conduct intake process with new family members within 24 hours of admission. This includes but is not limited to explanation of client’s rights and responsibilities, viewing of “Love and Life” video, and grievance procedures
  • Conduct psychosocial, as well as vocational/educational and housing assessments
  • Meet weekly with each family and develop and monitor family independent living plans
  • Maintain active independent living plans at all times and complete weekly progress notes
  • Provide child care referrals and assist families to navigate the NYC Department of Education system as well as ACS
  • Establish linkages and maintain solid working relationships with supportive housing providers, real estate brokers, managing agents, landlords and other service providers as resources for family members
  • Assess family readiness and barriers towards obtaining affordable permanent housing
  • Escort, prepare and assist family members in successfully identifying, securing and maintaining permanent housing
  • Accompany family members on medical, public benefit, and housing appointments, as necessary
  • Facilitate supportive group and individual counseling, workshops, psych education, and recreational activities
  • Conduct apartment inspections to assess consumer personal hygiene, room cleanliness, and overall ADL skills
  • Assist family members with accessing needed community services (medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network, referral and follow-up
  • Assist family members with problem solving and crisis intervention by addressing all emergency situations as appropriate
  • Act as advocate for family members as appropriate; encourage and support self-advocacy
  • Work as a member of the social service team in the development of on-site and off-site recreation, socialization, educational and skill building activities with a view towards community inclusion and integration
  • Participate in ongoing individual supervision, team meetings, case conferences and trainings
  • Adhere to on call system to facilitate crisis intervention, emergencies and situations that require immediate action
  • Maintain CPR/ First Aid and Mandated Reporter Certification
  • Maintain up-to-date, accurate, complete, and timely consumer chart documentation in compliance with agency standards
  • Knowledge of computer programs including e-mail, Microsoft Windows, Word, Excel, AWARDS and CARES
  • Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes
  • Assist in building evacuation during fire drills and building emergencies
  • Evening and weekend hours will be required

Qualifications

REQUIRED EDUCATION AND EXPERIENCE

  • Bachelor’s Degree is Human Services field required
  • Two years’ experience with homeless population. Knowledge of child and family development, Administration of Children’s Services (ACS), substance abuse issues, and housing
  • Ability to work with a culturally diverse population

PREFERRED QUALIFICATIONS & SKILLS

  • Familiarity of NYC public school system preferred
  • Experience in residential setting, group work, tenant organization, case management, referral, and/or community outreach strongly preferred
