Position Overview

S:US is seeking an LPN for our men's shelter in Brooklyn.

 

This position is located at 1000 Blake Avenue, Brooklyn, NY 11208

 

 

The LPN meets with the Program Director, Director of Social Services, Registered Nurse, and Social Service staff to coordinate shelter residents to receive ongoing treatment in the community. 

 

In this position you will;

  • Take vital signs, such as blood pressure, temperature and weight
  • Assist with diabetes medication and treatment 
  • Take medical histories and enter information in computer systems 
  • Coordinate with Case Managers, Senior Case Managers and Director of Social Services to ensure proper care, nutrition and treatment plans are followed 
  • Follow up with community health care providers to ensure medical treatment plans are documented, monitored and followed
  • Have flexibility to work a schedule that includes evenings and weekends 

 

Schedule: Tuesday-Saturday 10am-6pm

Qualifications

  • New York State Licensed Practical Nurse
    Experience working on a multi- disciplinary team and/or previous experience with homeless population
    Bilingual English/Spanish preferred but not required
